The reason why you read +1 for the hand 11 vs Ace in BJA3 in that the ill-18 are based on a S17 game.
Regarding your last question, yes and yes. You take the insurance and double down.
When you get BJA3, go to page 213. There, you will see that the index you need for doubling is -1, not +1, for H17.
