How to calculate insurance index?

I am playing around with the index generators for multiparameter counts from www.bjmath.com. I was wondering if anyone can explain how I calculate when to start taking insurance? The spreadsheets don't have this oh-so-important index number on them. I'm no math-phobe, so hit me with whatever it takes. Thanks!
