In shoe games, does Knock-Out work just as well as AO2? I know AO2 is better in handheld games.

Do you mean the playing indices? I don�t have them handy, but you could use the index generators to get them:

The playing indices are the same as above since they do not use the ace side count. The aces are only for betting purposes (and possibly multi-parameter playing adjustments). But using AO2 without the ace side count is going to make it perform fairly poorly. You would probably be better off using a simple system like HiLo (faster) or Zen (stronger) than using AO2 without the side count, especially in shoe games where the BC is very important.

By betting indices,I mean how many units to bet when the count is at +1, +2, etc.
for AO2 and HiOpt2 and Zen for that matter should all be roughly the same. Bet ramps vary by game rules, penetration, and exact wongout point, but for most games your optimal max bet will be between +7 and +12. The deeper the pen, bigger the base spread, and more aggressive the wonging the LATER the optimal max bet will be. Games with worse pen, smaller player spread, or a closer to play-all approach will cause the optimal top bet to be at an EARLIER true count. (with a corresponding smaller unit size)
Does anyone know the percent advantage you get with AO2? That is what added advantage do you get with +2, +4, ...

2.8% is the sum of the effects of removal for the small cards and 9 is the base, or sum of the point value tags of the Griffin2 count which was marketed as AO2. Peter Griffin's Theory of Blackjack should be part of your BJ library.
