Also splitting 4's 5's and 10's is not allowed, how much does this add to the house edge?

Go to www.wizardofodds.com and see what the different house rules add or takeaway from the house edge.
The only splits you would really make here are 4/4 versus 5 or 6. Of these two only the split against the 6 yields some additional ev but the situation is so rare that it doesn't actually influence the chances from the top. I think your 0.64% is a bit too pessimistic. It's rather 0.60% - or even better if resplit of aces is allowed.
